Fred's Story

Fred’s journey started in November 2023. I was tapped on the shoulder by Client Engagement Lead, Julia Wilson, “I have met a gentleman named Fred from Hamilton Hostel in Macquarie Hospital,” she said. “I think he would be a good fit for Macquarie Parkside.”

Tia's Story

We also used this time to build rapport with Tia and find her a place to live. After 6 months we found a home, but it needed significant work so our support team worked around the clock to transform it into a safe, purpose-built space to meet Tia’s complex needs.

Jon's Story

Jon's smooth 2-month transition from the hospital to an Achieve home reflects the care and collaboration of our team. With Jon facing such a big change, our team ensured Jon's transition was as comfortable as possible.

Geoff's Story

After weeks in hospital, Geoff* and his family needed proof that 24/7 clinical and behaviour supports could run smoothly outside a ward. Achieve mapped every task at his bedside, trained a specialist team and aligned his needs with a light-filled home in Sydney’s north-west.

Tim* had a solid routine at his Community and Lifestyle program but supports in his previous SIL house were winding down. His mum booked an inspection of an Achieve Australia home only minutes away. The visit happened to coincide with the house Christmas lunch.
